The internal check valve or o-ring was prolly shot. In CA, on pre-94 (OBD 2) vehicles... they have a machine at the smog test centers to test fuel cap function.
A massive EVAP leak like that will create a loss of vacuum with resultant decrease in air mass for combustion. To keep from stalling, the ECM will run the car in 'closed loop' mode with over-rich mixture and you will burn an assload of fuel.
91 huh... OBD 1 only. The idiot light wasn't even looking at that circuit. Too bad, so sad. I'm glad you got it figured out.
