Mine's a 2009 with 189k miles. I had no problems up until last weekend when the car started to shut down on the freeway and eventually had to have it towed to a local mechanic, then to a dealership.
The mechanic told me the 12v battery was completely dead and wasn't able to further diagnose so off it went to dealership. The dealership told me that I needed a new battery, ignition coil packs, an ECM fuse and reprogramming to get the car running. There were other wear and tear mechanical issues but not critical to issue at hand.
I plan to replace the battery, ignition coils and fuse (15 A mini) and have the dealer reprogram, if it's really needed.
I am trying to source the 12v from a place other than the dealers around me but not having much luck (see my first post re this matter).
Other than that the car's been driving well and getting about 32.5 mpg and noticeably a little louder enging on cold morning starts.
Other than regular oil changes, at 10k mile intervals, the only thing replaced were the rear struts and the EGR(?) valve.
YU888, what was the cost, parts + labor, for replacing the Hybrid battery pack with a refurbished one? In what city are you located?

I just hit 250k miles only 09 Altima Hybrid. Over the last cpl months code p0ac4 has popped and I reset it with my OBD II reader bought from Harbor Freight. The dealership wanted 1200 for the part i found a new one on Ebay for 250, took an hr to replace and code cleared and back to getting 33mpg. I did have to get the key remote reprogrammed after replacing module which cost me 90.

I have a 2008 and just crossed the 300k milestone. No major problems in the 10 ½ years that I have owned it. It has the ORIGINAL hybrid battery. I am about to treat it to new tires (I have over 110k on the set I am replacing) and front brakes (pads only) – this is only the second time since owning it that I have done anything to the front brakes. I do all factory service and the car will have its 10th major service this week. The 12v battery has been replaced twice, a new exhaust system including catalytic converter is the most expensive maintenance that has been done. I am still getting 33 -34 MPG and drive the car very hard. By far the best, most reliable car that I have ever owned.

@tspeed - sorry for the late reply, I haven’t checked the board for a while as you can tail. The costs I looked at for replacing the battery pack were $999, or $1899. The first was a standard refurbished battery where sells were balanced to the strongest cell in an already used battery. The second was I lightly used battery that had been rebalanced by replacing any bad cells. I chose the second because the donor vehicle only had 60,000 miles. There are more options nowadays, but you probably need to be in larger metropolitan areas to find them. For example in Southern California, there are services wear under $2000 they can get you a three-year warranty battery. That said, I’ve been extremely happy with mine.
currently about 230,000 miles, the drive is still pretty phenomenal. Quiet smooth and reliable. Mileage is staying around 30 with fairly aggressive driving. I did get some suspension work done as the front suspension was pretty shot so I replaced control arms and struts. Drives like a new car .

@cali - thank you for your post. The fact that you hit 300,000 miles is very encouraging to me as this is the first time I’ve actually considered keeping my car to meet that mile stone. ! While I do get some rough for cold starts, that issue has always played this car, sounds like a diesel truck in extremely cold weather but nonetheless reliably will run and run well

So I haven't seen much in higher mileage Altima Hybrids so I figured I would start a thread and see how other NAH owners are doing with their cars.
I have a 2007 model with over 185k miles. I just replaced the battery pack last week. It is still one of the smoothest easiest to drive cars I have had and I still love driving it. Mileage is in low 30's these days with mixed city hwy but can still sometimes get 600+ miles per tank.
I have done regular maintenance on a fairly aggressive schedule with 0-20w synth throughout myself (except two or three dealership changes that for some reason used 5-30.. Every 30k service was done at dealership or hybrid specialist so HEV transmission services and such were done there.
Overall no major issues except a leaky oil filter housing.
Cosmetically doing well with only small normal scuffs and dings, and rock chips. Interior doing ok as I upgraded to leather seat covers.
Anyhow any other updates and issues would be appreciated as I am really hoping to keep this one for a few more years so 200k plus should be in the cards. Thanks for your input.

I have 203K plus mileage on my 07 Altima Hybrid, All was well until I took car to the dealership to get a full diagnostic on it, other than minor mechanical issues I got clean bill of health, one week later 2 coils went, I replaced them but since then I've been experiencing rough idling. I have changed the manifold and flex pipe as well as the torque mounts. Car still runs, still gets 600 miles a tank but front end suspension is always an issue, especially in New York. The only leak that has been detected is the oil cooler seal seeps but its not major, probably change that the next time I do a radiator flush. My hybrid battery is still strong, I have changed the regular battery 2 years ago.
